Before a student can be accepted at Foundation Christian Academy, they are required to submit the following documentation: 1. A record of up-to-date immunizations 2. A completed physical exam 3. Complete transcripts from previous school(s) 4. Completed registration packet with appropriate fees 5. Entrance/Placement Screening (K-8th) 6. Personal recommendations K-2: One recommendation 3-5: Two recommendations 6-8: Three recommendations 7. Written personal testimony (Grades 5-8) 8. Birth Certificate (Required for all new students) 9. Parents also must submit a personal recommendation
